Personalised Planning: Our support coordinators work closely with the NDIA Hospital Discharge team to develop individualised discharge plans tailored to your unique needs.
Communication with Healthcare Providers: We liaise with hospital staff and health liaison officers (HLOs) to ensure that all necessary information is gathered for your discharge, including personal care and daily living supports.
Home and Living Assessments: If needed, we assist in arranging home and living assessments to determine the best living arrangements and supports for you.
Access to Essential Supports: We help you navigate the NDIS process to access necessary supports, including Specialist Disability Accommodation, if required.
Post-Discharge Follow-Up: Our support coordinators engage with you and your support network to ensure that all services are in place and functioning effectively as you transition back home.
The Discharge Process
Our streamlined discharge process includes:
- Notification to the NDIA upon your hospital admission.
- Timely contact from our team within 4 days to discuss your needs.
- Collaboration with hospital staff to gather essential health information.
- Conducting planning meetings to finalise your NDIS plan and supports.
- Ongoing support to implement your plan and ensure a successful transition home.
